This form can be completed digitally, using Adobe Acrobat (available for free download online), or it may be printed and completed by hand. Please email the completed application to the Office of Programs and External Relations at i-house-programs@uchicago.edu. To facilitate marketing and the creation of publicity materials, we recommend that this application is submitted at least two weeks prior to your virtual event.
